Setting Up an Account

Our dealer application is available below. The application form is set up to be filled out right on your computer, and many of the fields are auto-fill, please be our guest to use this feature.

Please complete and return the forms as soon as you can, so we can get your account up and going. Please include copies of all applicable documents listed below:

• Photos of your "brick & mortar". Store front or field photos.
• USA - State ID Number – Copy of your State Resale Certificate and/or Business License
• USA - Federal ID Number - EIN Number (Employer Identification Number, issued by the IRS) or your Social Security Number if your business is a sole proprietorship.
• USA - Tax Exemption certificate (A blanket form is attached to the application if yourstate does not provide you with one)
• Canada – Copy of your provincial business license & your Canada Revenue Agency Business number (GST number)
• Europe – VAT Number & a Copy of your business license
• All other countries: Please supply us with a valid business license for your country of business or other official document to show your active working business.

Once your forms have been submitted, the information will be forwarded to our sales team, who will contact you about our product line and provide you with a price list, and go over any additional questions you may have like ordering or logistics.
